Attention!

These procedures should be used only if AMI's Tech Support cannot resolve the problem you are encountering.

Retain these procedures and forms for future reference. You must complete the form in its entirety. Please select the type of RMA that you want to submit:


Online RMA Form for StorTrends Storage RMA Products.

Online RMA Form for All Other AMI Products.

Request an RMA Form Faxed to you.

Failure to complete the RMA Request Form and follow the procedures below will delay processing of your request and repairing of your board(s).

Procedure

  1. Ship to: Who will receive the complete board. Be sure to list the name of a contact person who reported the problem.

  2. Serial Number: The serial number is listed on the AMI invoice. It may also be located on the motherboard. It is a ten (10) digit number over a barcode. This must be on the request.

  3. Configuration: Briefly describe how the board is configured, include peripherals and software applications or any special setups.

  4. Other Items Sent: List any controllers, video cards, SIMMs, etc that you are sending with the board for testing purposes.

  5. Problem Description: Briefly describe the symptoms of the problem(s) you are encountering. You may include a detailed description with the board. Include board configuration, peripherals and software applications.

  6. Bill To: If Product is out of warranty, you will be contacted for billing information. Non-warranty repairs will be billed. Charges are based on a $100.00 minimum fee, plus parts and shipping. Should the repair costs exceed $200.00, you will be notified for prior approval before repairs are complete. Average costs of repair is $150.00.

  7. Method of Shipment: Normal RMA repairs are shipped on a two (2) day service. Non-warranty repairs will be charged freight and any special shipping requests will be paid in full by the recipient.

  8. Cross-Shipping: Some StorTrends storage products come with a cross-shipping policy. After you receive a replacement part, you must ship the suspect part in the replacement packaging. If you do not, you will be charged for the replacement part. Hard drives that are packaged improperly are easily damaged during shipping.

  9. Method of Payment: American Megatrends Inc. offers a one (1) year warranty on all hardware products unless otherwise documented in a legal contract with the customer. This applies to the original purchaser only. For non-warranty or out-of-warranty repairs, AMI will accept: company purchase orders, credit cards (American Express, VISA, Mastercard, & Discover) and cashier’s checks.

Upon receipt of the RMA Request Form, a RMA number will be processed and issued to you. Please allow at least 24 hours for a response.

Your RMA number will be returned to you via email with notification of the status of your board, i.e. warranty or non-warranty. Remember, non-warranty boards will incur a minimum charge of $100.00 plus parts and shipping.


Please retain a copy of your notification for your files and future references.


Shipping Your Product(s) to AMI


Mark the outside of your box with the RMA number. Failure to do so will delay receiving and processing your board(s). Be sure to pack your board in an anti-static bag with plenty of packing material to ensure no damage. Include a copy of the original RMA request, with problem description, inside the box or attached to the board.

Please allow at least ten (10) days before inquiring about the status of your board(s). Please inspect and test your board(s) upon receipt. There is a thirty (30) day warranty on all repairs.
